Identifying Challenges and Solutions for Indirect Comparisons in Myasthenia Gravis

Generalized myasthenia gravis (gMG) is a rare condition characterized by muscle weakness due to reduced connectivity between nerves and muscles throughout the body. There are many newer treatments for gMG, but no clinical trials directly comparing them. Comparative effectiveness can instead be estimated using analyses known as indirect treatment comparisons (ITCs). There are many ITC methods with varying requirements, so a feasibility assessment is an important first step to identify any challenges in conducting ITCs and consider how to address them.

In the first publication of its kind in gMG, Nils Erik Gilhus, MD; Saiju Jacob, MD; Brian Hutton, PhD; the team at Johnson & Johnson (Kavita Gandhi, Mahmoud Hashim, Suzy Van Sanden); and the Value & Evidence team at EVERSANA (Chris Drudge, Anna Nero, and Sumeet Singh) assess the feasibility of ITCs for gMG treatments by examining the comparability of 15 clinical trials. The assessment identifies several important challenges that could lead to unreliable ITC results, including key differences between trials in patient populations, how treatments were administered, and when treatment outcomes were evaluated. The research team concluded that it is important to use multiple ITC methods and outcome timepoints to reliably assess how gMG treatments compare with each other.
